It is filling gaps in its knowledge with outputs that are statistically likely rather than actually true, and presenting them as fact.
This is one of the most common failure modes in generative AI, and one of the hardest to catch because hallucinated content looks exactly like accurate content. It is well-written, well-structured, and delivered without any disclaimer.

Language models generate text by predicting the most likely next word based on patterns in training data. They do not have a concept of truth.
